Tell Your Story

A group of family members, self-advocates, providers and association representatives have been participating in a group called the Stakeholder Policy Forum (SPF) which has asked us to share the survey below.   It’s not your usual survey, in that respondents first tell a story about an experience they have had regarding OPWDD services, and then answer a series of questions about their story.  The hope is that trends, issues and ideas will arise that will assist OPWDD in future policy making -- OPWDD has participated in the development of the survey questions.

About "Your Voice Matters"

This survey is about services for people with Intellectual and/or Developmental Disabilities (I/DD). To get started click here.

We welcome stories from anyone. 

Please tell us your story, and then answer some questions about what that story means to you. This takes most people about twenty-five minutes, though some finish in as quickly as ten minutes. To get started click here.

This survey was put together by members of the self-organized Stakeholder Policy Forum, a group that got started during the Covid-19 pandemic.

We created the survey with help from the NYS Office for People with Developmental Disabilities (OPWDD). The survey opened for input on August 20th. We will keep it open for up to 90 days. We plan to report-out to the community in late September.
