DESCRIPTION: The process of transferring youth with special health care needs to the adult
medical practices can cause anxiety and stress to both youth and their parents. During this webinar
we will discuss strategies for preparation (transition) and provide tools to use to make it a smoother
process.
WHO SHOULD ATTEND: Parents, family members, caregivers and professionals who want to enhance
the lives of children, teenagers & young adults. We welcome anyone who strives to achieve the same
